· call 780-702-4486 (Customer Call Centre) to book an appointment

Thursday:
Before noon only
· complete the Dynacare Payment Authorization form – this test is not covered by Alberta Health Care
· bring the completed requisitions (listed below) and the payment authorization form to their laboratory appointment

- Handling/Storage Comments:

Storage: Specimens must be kept at room temperature
Transport: Do not ship Friday to Sunday or on holidays

• This test is not covered by Alberta Health Care.
• Do not charge the patient for the Harmony kit - the company is billed directly (CO)
