Electronic order (EPIC, Meditech) or Transfusion medicine requisition is required

Collection Instructions:

• Requisition and specimen must be labelled with patient's FULL first and last name
• Preprinted labels can be used if the patient's full name is visible. If the name is cut off and not corrected then, the specimen will be rejected
• Specimens must be collected using the Transfusion Service Identification Number (TSIN) or Blood Bank Identification Number (BBIN) system as appropriate to the collection site
