Testing Prerequisites:
DynaLIFE sites only perform the collection of this test - DynaLIFE does not perform the testing

Test collection and the performance of the test is not covered by Provincial Health Care plans

The testing facility or the patient will be charged a collection fee

Choose a paternity testing facility listed below or choose one of your own:
•  Canadian DNA Services
•  Maxxam - the DNA Lab

Patient Instructions:
FIRST:   
Contact your chosen company for  a requisition and any information regarding the testing and fees incurred

If using a suggested testing facility…

If using a testing facility of your choice…

then, a collection kit:
• 
is available from the testing facility 
or

•  can be obtained from a Patient Care
    Centre
(PCC) stocked with kits specific to
    the testing facility used

then, a collection kit is available from the testing facility

THEN:
Book an appointment through the DynaLIFE Customer Care Centre at 780-702-4486 or 877-702-4486

If using a suggested testing facility…

If using a testing facility of your choice…

then,

•  bring the requisition to the PCC that
    stocks
the kits specific to the testing
    facility selected    

    or

•  bring the requisition and the collection

    kit (from the testing facility) to the PCC
    where
the appointment was booked

then,

•  bring the requisition and the collection
    kit
received from the testing facility, to
    the PCC 
where the appointment was
    booked

•  bring payment (credit or debit card,

    only)
