Ordering Information:
Testing includes:
•  Lipid panel: total cholesterol, triglyceride, HDL cholesterol, LDL cholesterol, non-HDL cholesterol
•  10-year Framingham risk score
•  Statin recommendations and target lipid values based on Canadian Cardiovascular Society Dyslipidemia guidelines

Collection Device/Tube Type:
1. EDM Base Lab: 5 mL blood - GOLD SST (gel)
2. CGY DSC: 4.5 mL blood - MINT LIHEP PST (gel)
3. NOTE: The preferred tube type at each testing site is acceptable at the alternate site
Collection Instructions:
Ordering provider:
•  The CVD risk assessment includes the lipid panel. Do not order a lipid panel with the CVD risk assessment.
•  Answer all questions in Connect Care order entry or clearly record the answers on the requisition. The patient history information is REQUIRED for calculation of the Risk Score; if all required information is not provided, only a lipid panel will be reported.
  Systolic blood pressure (BP)
  Tobacco use
  Treated for high BP
  Diabetic
  Chronic kidney disease
  Atherosclerosis
  Family history of CVD in a 1st degree relative (male < 55 years and/or female < 65 years)

Laboratory Staff:
All information on the requisition must be entered. However, laboratory staff will not inquire about missing or illegible information.

Submission Requirements:
1 mL serum or lithium heparin plasma

Frequency of Testing:
Daily
Comments:
•  This test includes calculation of the Framingham Risk Score (FRS), the preferred dyslipidemia screening tool of the Canadian Cardiovascular Society and provides an estimate of the 10-year risk of a cardiovascular event based on the results of a lipid panel and patient history
