State specimen source
State current antibiotic and its start date
Provide clinical history including symptoms and signs, their duration, and suspected diagnosis
Indicate if chronic infection, sinus tract or fistula present
Indicate any underlying conditions such as diabetes, steroid, immunosuppressive therapy, malignancy, HIV, etc.
Collection Device/Tube Type:
Sterile 60 mL plastic container (orange-top).
Comments:
Non-bacteriostatic saline is required
Collection Instructions:
1.
Collect tissue specimens under aseptic conditions
2.
Avoid any contamination while placing tissue into specimen container
3.
Place tissue in a sterile plastic container on sterile gauze. Moisten with non-bacteriostatic saline
4.
For multiple specimen collections, ensure each specimen container has a corresponding requisition
Note: 1. Tissue biopsy/samples should be collected from areas within and adjacent to the area of infection 2. Large enough tissue samples should be collected to perform the tests required (i.e. 3-4 mm biopsy sample) 3. Tissue submitted in a dry, sterile container are sub-optimal and will compromise the recovery of fastidious organisms 4. Avoid swab collection if biopsy samples can be obtained as viable infected tissues are always superior to a swab
