Always provide:
•  specimen source
•  clinical history - signs & symptoms, duration, suspected diagnosis
•  Indicate if chronic infection, sinus tract or fistula present
•  Indicate any underlying conditions - diabetes, steroid immuno-suppressive therapy, malignancy, HIV, etc.

Collection Device/Tube Type:
60 mL sterile, plastic specimen container
Anaerobic collection device, e.g. Port-a-cul/cult
Comments:
Collection Instructions:
  1. Collect tissue specimens under aseptic conditions
  2. Avoid any contamination when putting tissue into the specimen container
  3. Small tissue:  Embed tissue into an anaerobic transport medium within 5mm from the bottom.
