Always provide/record:
•  specimen source - specify left or right eye
•  current antibiotic therapy and start date
•  clinical history - symptoms, signs, and their duration
•  If filamentous fungi/mold culture is required it must be recorded on the requisition

Obtain specimen for culture before topical agents are applied
